Key Components Include...

• Direct Provider Agreements through various medical partners such as: Accountable Care Organization (ACO), Independent Physicians Association (IPA), Medical Service Organization (MSO) work together to provide high quality, for low-cost access to your physician.

 

• Third-Party Administrator (TPA) set-up and administrate employer-sponsored health and welfare benefit trust.

• Patient/Physician Cooperatives (PPC) is a licensed, discount, medical organization that works with different medical organizations and vendors to provide a cost-effective healthcare experience.

• Prescription Benefit Management (PBM) is a strategic consumer driven pharmacy program that offers members low prices for prescriptions.

• Association Insurance Policies are community rated insurance policies. These include Catastrophic, Indemnity, and Lump Sum Cancer coverage.

Maximum Value Plan 500

Maximum Value Plan 500 (MVP) is a comprehensive, self-funded, major medical plan providing a co-payment only benefit structure to the beneficiaries of an employer group.

 

The MVP500 plan includes access to the PPC Network, PHCS Provider Network, Catastrophic Coverage, SimpleSaveRX, Lab Testing, Diagnostic Imaging, Teladoc, and Doc Wellbee discounts for dental, vision, hearing, and roadside assistance.

Minimum Essential Coverage Plans

Minimum Essential Coverage (MEC) Plan is structured to meet your employee’s basic healthcare needs. MEC benefits include access to the PPC Network, PHCS Provider Network, SimpleSaveRX, Lab Testing, Teladoc, and Doc Wellbee discounts for dental, vision, hearing, and roadside assistance. Additionally, you may choose a PPC Provider at $0 co-payment or nominate your current primary care physician for approval with PPC. Finally, you may select the Healthcare Reimbursement Account (HRA) debit card to direct your benefit.

 

MEC Plus Plan structure and benefits are the same as the above MEC Plan. The MEC PLUS Plan added benefits include Diagnostic Imaging at $0 co-payment at participating imaging centers. Typical services provided are X-Ray, CT Scan, MRI, PET Scan, Ultrasounds, and Mammograms.

Association Insurance Benefits...

Hospital Indemnity Plan

This guaranteed issue hospital plan provides payments for inpatient/

outpatient physician care, facility costs, intensive care, skilled

nursing, substance abuse, and treatment of mental illness, all without

Lump Sum Cancer Insurance

Provides a $25,000 benefit upon the first diagnosis of cancer. The cash benefit is a crucial component of the member’s healthcare plan. This allows recovery to

be the sole focus, rather than anxiety and stress over financial

worries.

 

This plan provides an additional $25,000 benefit for breast cancer and supports the Susan G. Komen Foundation. Request Summary of Benefits.

Catastrophic Coverage

Have peace of mind in the worst situation. PartnerRE stop-loss

insurance is catastrophic coverage that covers 90% of medical and

hospital expenses that exceed $50,000 and up to a maximum of $5,000,000. Request Summary of Benefits.
