This is a Health Care Cooperative between patients and physicians that includes guaranteed pricing for health care at Fair and Reasonable Rates for members using the cooperative’s member physicians and facilities.

PRIMARY CARE FROM SELECTED PCPs

Primary or basic medical care services are provided to members with no co-payment and no health qualifications. Choose from 49 primary care physicians in 57 different clinic locations. Each member signs a monthly payment plan agreement with primary care provider through which the availability of physician and his or her service are paid in full. Once enrolled, members may visit their selected primary care physician as many times as needed.

DIAGNOSTIC IMAGING

We offer plans that give you access to three imaging locations. You can go to Main Imaging or the two locations for Gulf Coast Cancer and Diagnostic Centers. You will be covered 100% with no copay, deductible, or out of pocket expense. They will be able to perform any of the tests that are listed on their detailed benefits sheet that include X-ray, MRI, CT Scans, Ultrasounds, and more.

LAB TESTS

PPC exclusively partners with Clinical Pathology Laboratories (CPL) to provide you regular screenings and Lab Tests covered at a 100% with several locations in Houston and surrounding areas.

DENTAL DISCOUNTS

we will refer you to a dentist which will only charge you the exclusive rate for the visit and any procedures you may need. You will be given a preferred rate that is pre-agreed and lower than normal rates.

We have pre-agreed pricing that is exclusively for our members. We will refer you to a participating provider and you will pay the agreed rate at the time of service.

PATIENT ADVOCACY

PPC will allow you peace of mind necessary to deal with important matters by providing Patient Advocacy to properly guide you through the system and its red tape by mediating and negotiating medical bills and expenses on your behalf.

GROUP HOSPITAL INDEMNITY INSURANCE (NEW ERA LIFE)

Hospital confinement indemnity coverage of $1,500 a day is designed to provide you with a fixed daily benefit during periods of hospital confinement resulting from a covered injury or sickness. Pays Specialist s based on the PPC Rate Sheet Less a $25 CoPay. Maximum Covered Benefits of $25,000 per person per calendar Year. 60-day waiting period for sickness and 270-day waiting period for Pregnancy.

Association Group Insurance

Accidental Death $ Dismemberment Benefit

Pays the beneficiary up to the benefit amount listed for the member’s death or loss of certain body parts in a covered accident or a portion of that amount for the accidental death of a family member, if this is a family membership (50% for covered spouse, 60% if there are no dependent children; 20% for covered dependent children, 25% if there is no covered spouse)

Accident Medical Expense Benefit

Pays part of the expenses you are charge by the hospital , doctor, or certain other charges, up to a maximum of the amount listed if you are injured in a covered accident. Subject to a $100 deductible.
